Out-of-Network PT Provider

• I am an Out-of-Network Physical Therapy provider. It is the client’s responsibility to speak with their insurance carrier to confirm out-of-network and telehealth insurance benefits.

• Payment is due at the time of service. A credit card is required to hold virtual appointments.

• I provide a superbill for each session that can be submitted to the client’s insurance carrier for reimbursement.

• Clients can use a health savings account (HSA) debit card for payment.

• Fees for each service are listed in the online booking portal.
